This application form must be completed in full by all students who are applying for any short course at CSI Academy. Once this application form is completed and the supporting documents are attached, it must be submitted to CSI Academy. Your application is subject to review and approval. Once your application had been accepted and processed the course administrator will issue you with a student number.

PAYMENT METHOD
Course fees are payable in full prior to commencement of the course.
Monthly instalments:
50% deposit (R5250) is mandatory before 20 February 2024.
Minimum amount of 10% (R1050) can be settled monthly after the initial 50% settlement.
Note that no certification will be awarded if fees are not settled in full.
Failure to comply will result in cancellation of your training profile.
If your employer is paying your course fee:
– A letter must be obtained in which the employer commits to paying the course fee
– An invoice will then be sent to the employer by the course administrator
